The lighting was 3 Alien Bees B800 strobes and a reflector. Two strobes were on the model's left side and feather around her with large softboxes about 3 feet from the subject. You're correct, the main light was not much higher than eye level. She is a rather tall girl and we were working with a low ceiling and she was standing. There was a large 54 inch reflector on the model's left side slightly in front of her. The lighting was set up to wrap around her. The third light is a strobe above her with a 20 degree grid, about 90% of the hair light was actually falling on the floor. The main light was f11, the other softbox was f9.5 and the hair light was f8. I actually used an f8 exposure on the shot, I was over-exposing to get a slightly washed out look and give her eyes some natural pop. I do use a single strobe and a couple of reflectors often. I just posted some new pictures from a session on Father's Day using a single B800 strobe and reflector. You can view them on my blog
